5's on Slug Called Slyma Vol 1, original release doesn't seem to be listed on Discogs, dunno if it's on the '96 release?
Mate's copy in '92/93 had a cartoon slug on the inner bit of the vinyl if memory serves.....

EDIT - yeah, here you go, this is the puppy...released in '93 according to htfr.....
